OLE Health Recognized as Elite Health Care Provider

Napa Valley’s Premier Community Health Center Recognized as PCMH by National Committee for Quality Assurance.

OLE  Health has received Level-3 recognition as a Patient-Centered Medical Home (PCMH) by the National Committee for Quality Assurance. Earning this coveted recognition places OLE Health as one of only two medical practices (or health care systems) in the Napa Valley Region to receive this national accolade.

A Patient-Centered Medical Home is a model of care that strengthens the provider-patient relationship by replacing episodic care with a coordinated and long-term healing relationship. Each patient has an ongoing relationship with a personal primary care provider, who leads a team at a single location that takes collective responsibility for patient care, meeting the patient’s health care needs and even arranging for appropriate care with other qualified clinicians. A PCMH provides a more personalized, coordinated, effective and efficient health care experience.

They also enjoy convenient online access to their care team, including obtaining test results or scheduling appointments. The PCMH designation is valid for a period of 3-years before renewal.
