About Kathleen “Grace”

Grace started at CommuniCare+OLE in 2023. She received her Doctor of Nursing Practice in health systems leadership, with clinical certification for family practice and nurse-midwifery, from Emory University in 2018 and completed her Bachelor of Science in nursing at Northern Illinois University. Prior to coming to CommuniCare+OLE, Grace has always been committed to serving vulnerable communities working in Chicago’s West Side, rural Georgia, and socially disadvantaged neighborhoods in Minneapolis and northern California. Recognizing that there is more to health promotion than providing medical care, she also received her Master of Public Health to give her a greater consideration for how to assess and meet patient needs. She specializes in preventive wellness, obstetrics, mild illness, and chronic conditions across the lifespan – from newborn until end of life.

My approach to care

My care philosophy/approach to care is to understand your priorities, health goals, and concerns while thinking about other responsibilities in your life that may be more difficult to control. I want you to enjoy your life for as long as possible, and I discuss the options for self-care, medical care, and possible complications that can affect how you feel and live.

Why I went into healthcare

I became a nurse practitioner because there are many scary things in this world, and I want to make a safe space for people when something scary comes along.
