Perinatal Patient Care Services Coordinator

JOB TITLE:               PERINATAL PATIENT CARE SERVICES COORDINATOR

REPORTS TO:           Back Office Supervisor

OVERVIEW:          The PN Patient Care Services Coordinator works closely with the perinatal care providers and is responsible for coordinating the care of perinatal patients at CommuniCare+OLE. The PN Patient Care Services Coordinator works with a team of perinatal providers, community health workers, and other staff to ensure the coordination and quality of services provided.  CommuniCare+OLE is a Federally Qualified Health Center providing comprehensive patient-centered primary care services including medical, dental, substance abuse treatment, behavioral health, and outreach services to low income, multi-ethnic, uninsured, and underinsured populations.

LOCATION: West Fairfield, CA
PAY RANGE: $23.22 to $31.53/hr

SCHEDULE:                  Full-time 40 hours per week, Monday through Friday between the hours of 8:00am and 5:00pm. Office located in Fairfield. Some evenings and weekends may be required.

RESPONSIBILITIES:

1.   Responsible for the coordination of care for perinatal patients including but not limited to: scheduling of patient appointments, referrals to specialists and labs and tracking of patient attendance to referrals and appointments; coordination of outpatient/inpatient continuity of care; assist clinicians with tracking of high risk patients and tracking of abnormal test results; schedule care transfers, hospital admissions, and referrals as directed by the provider;  monitor referrals to available resources including childbirth classes, WIC, Substance Abuse treatment, Social Services, Dental services, nutritionist, etc.

2.    Communicate effectively with Medi-Cal, Partnership, and other insurers to assist clients with obtaining health care coverage and problem-solving insurance issues as needed.

3.    Adherence to CPSP guidelines to ensure appropriate health education, nutrition and psycho-social care services are offered and a plan of care has been developed and followed up for each client.

4.    Assures the consistent delivery of high-quality services through thorough patient chart preparation including; all lab, ultrasound and medical reports are in place; consistent chart order; accurate completion of routing slip; preparation of lab referrals and scheduling for health education, nutrition and social worker as needed.

5.    Responsible for the accurate completion and timely submission of billing records.

6.    Ensure compliance to quality assurance in accordance with written policy and procedures. Including abnormal lab tracking, DNKA logs, high risk patients, and post partum patients.

7.    As needed, provide one-on-one health education to perinatal clients attending the program. Provide initial patient intake including medical history, financial screening, and lab forms.

8.    Conduct audits and QI chart review as needed.

9.    Attend clinic staff meetings, PN staff meetings, and other operations meetings as needed.

10.  Perform other program duties as required to offer high quality, accessible Perinatal services as assigned by the Perinatal Administrator and Perinatal Services Director.

11. Other duties as assigned, including filling in for Medical Assistants as needed when available.

 

MINIMUM POSITION REQUIREMENTS:

Education:  Medical Assistant or college level coursework with clinic/medical knowledge gained through a combination of training and experience.

 

Experience:   Experience working in a clinic setting and a demonstrated commitment to comprehensive care for low-income women. Administrative experience required. Bilingual English/Spanish preferred. Demonstrated knowledge of local health care delivery system. Experience working in prenatal and postpartum care preferred. Demonstrated ability to assume responsibility and to work independently as well as in a team. Experience working in a busy environment, often under pressure, while maintaining accuracy, attention to detail, and courtesy to clients, other employees, and customers. Demonstrated commitment to high quality health care for low-income populations. Experience with meeting budget guidelines and controlling costs strongly preferred.

 

Expectations:  Attendance and punctuality is necessary. Ability to work well with a multidisciplinary team.  Ability to problem-solve and demonstrate self-motivation. Must be able to type and use a computer efficiently, at an intermediate level. Must have a commitment to excellence and high standards including but not limited to excellent written and oral skills, strong organizational, problem-solving and analytical skills. Must have ability to manage priorities and workflow. Required to have versatility, flexibility, and willingness to work within constantly changing priorities. Acute attention to detail required. Expected to have strong interpersonal skills and have the ability to understand and follow written/verbal instructions. Able to work independently with little or no supervision. Required to have the ability to deal effectively with a diversity of individuals at all organizational levels and with external customers. Must be able to drive and have a car for use in the position.

Work is performed in a typical office environment with moderate equipment noise. Regular travel to all agency sites required. Able to provide proof of vehicle insurance and valid CA driver’s license.
