About CommuniCare+OLE

Established in 2023, CommuniCare+OLE is the result of a union of two health centers with deep roots in their respective communities and reputations for providing high-quality primary care to all, regardless of insurance or ability to pay: OLE Health of Napa and Solano Counties and CommuniCare Health Centers of Yolo County. Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY: The Welcome Baby (WB) Registered Nurse (RN) provides in-home nursing care to postpartum Yolo County residents and their babies, including physical assessments, COVID vaccination, screenings, education, and connection to primary healthcare, behavioral healthcare, and community resources. The WB RN provides telephone follow up for WB clients as needed. The WB RN is trained as a primary care RN with specialty training to perform WB home visits, and fills-in as a primary care RN as needed and assigned. The WB RN works as a team with the WB Coordinator, the WB Community Health Workers, the CHILD Project: Road to Resilience (R2R) staff, and CommuniCare+OLE clinic managers, providers, and support staff to provide quality, coordinated care to WB and CommuniCare patients. The WB project consists of postpartum registered nurse (RN) and community health worker home visits, Yolo County home visiting collaboration involvement, and expansion of CHILD Project: Road to Resilience (R2R) services. WB services are provided for Yolo County families that are uninsured or have Medi-Cal insurance. WB is a partnership project between First 5 Yolo County, CommuniCare+OLE, and the Yolo County Children’s Alliance. This is a temporary position; continuation of this position past October 2024 is not guaranteed.

EDUCATION, EXPERIENCE, TRAINING

  1. Completion of an accredited RN Program
  2. Current California RN license and good standing with the Board of Registered Nurses required
  3. CPR Certification required
  4. At least 6 months of experience providing nursing care to the perinatal and/or pediatric population required
  5. Previous experience providing lactation assistance as an RN required
  6. Current lactation certification and/or ability to achieve lactation certification within 6 months of hire.
  7. Bilingual English/Spanish preferred

DUTIES AND RESPONSIBILITIES

 
1. Provides and manages patient care for Welcome Baby clients in their homes under the direction of the  CommuniCare+OLE Chief Medical Officer and Director of Nursing, including physical evaluations, assessments, and diagnoses as appropriate for WB clients and their baby(s). Consult with staff medical providers at CommuniCare+OLE and providers from Welcome Baby healthcare partners as needed.
2. Provides and manages patient care for CommuniCare+OLE primary care patients in clinic settings under the direction of staff medical providers and the CommuniCare+OLE Director of Nursing.
3. Utilizes the nursing process (assessment, diagnosis, planning, intervention and evaluation) in patient care and daily activities.
4. Provides patient education to WB clients on essential topics, including but not limited to infant care (feeding, sleeping, cues, bathing, umbilical cord care, etc.), immunizations, postpartum mood and anxiety disorders, sexual activity and birth control, when to call a medical provider, and babywearing.
5. Participates in lactation training and maintains lactation certification throughout employment. Provides lactation assistance to WB clients. Partners with CommuniCare+OLE IBCLC and other resources to provide ongoing lactation support.
6. Administers the COVID-19 vaccination to WB clients as needed.
7. Schedules WB clients for medical visits, including but not limited to postpartum, well baby, establish care, and sick visits.
8. Processes clinical patient information and WB data collection requirements in patient electronic health records (EHR).
9.  Participates in quality improvement projects.
10. Assists with coordination of specialty care for WB clients as needed.
11. Assists with meeting the needs of patients with complex medical problems, including facilitating referrals and follow-up care as needed.
12. Assists WB clients with coordination of care to other medical services, including dental care, behavioral health, diabetes education, etc.
13. Attends clinic and general CommuniCare+OLE meetings as required.
14. Other duties as assigned.
