LOCATION:                        This position is primarily located in Yolo County (Davis, Woodland, West Sacramento) with                                                         the occasional travel to Napa or Solano county.

Building on a legacy established by both organizations in 1972, CommuniCare+OLE is a network of federally-qualified health centers with 17 sites across Napa, Solano, and Yolo Counties. It offers comprehensive care, including medical, dental, behavioral health and substance use treatment, nutrition, optometry, pharmacy, care coordination, referrals, and enrollment assistance to more than 70,000 individuals, and no one is turned away due to lack of insurance, immigration status, or ability to pay. Many services are offered outside of its sites, including mobile health, home visiting, and community and school-based programs.

JOB SUMMARY: The Psychiatric Nurse Practitioner works on a multi-disciplinary team supporting Psychiatry services imbedded within primary care sites across counties (Napa, Solano and Yolo) and may include virtual visits to minimize travel.

 

EDUCATION, EXPERIENCE, TRAINING

 

  1. Graduate of an accredited Master of Science in Nursing required 2 years minimum working in the behavioral health/mental health field
  2. Current California Nurse Practitioner certification required
  3. Current Board certification as Psychiatric Mental Health Nurse Practitioner preferred
  4. Current California medication furnishing license required
  5. Current DEA License required
  6. Current Basic Life Support CPR certification required
  7. Experience in a community healthcare center preferred
  8. Bilingual (English/Spanish) language preferred
  9. Must be able to adapt and multi-task to manage a high volume of patients

 

SECTION I:   PERFORMANCE ACCOUNTABILITY

 

  1. KNOWLEDGE OF WORK
1. Knowledge of crisis intervention, applicable laws and ethical consideration related to patient rights,

treatment, safety and confidentiality.

2. Knowledge of psychotropic medications and physical health conditions/chronic disease
3. Knowledge of the current Diagnostic Statistical Manual of Mental Disorders and ability to accurately use this knowledge to diagnose patients and inform treatment planning.
4. Knowledge of Federally Qualified Health Centers and the key health center principles and guidelines

 

  1. DUTIES AND RESPONSIBILITIES
1. Provides comprehensive psychiatric assessment, diagnosis, prescription of drug therapy, planning of treatment, and evaluation of therapy
2. Provides brief therapy-based interventions as needed
3. Prescribes psychotropic medications based on clinical indicators. Review medication(s) that patient is currently taking, prior to prescribing to prevent adverse reactions.
4. Provide patient education and advice regarding side effects and dosage adjustments. Monitor/evaluate pharmacological treatment progress.
5. Orders appropriate lab tests and interpret results. Review and manage patient information including labs, prescription refills, outside consultation notes, previous medical records, specialist referrals, etc
6. Provides feedback and consultation to primary care medical providers and other treatment or service providers in the community regarding the behavioral health and/or psychiatric aspects of the patient. Offer specific recommendations related to integrated treatment planning
7. Prioritizes referrals and schedule appointments, providing crisis intervention and follow ups as needed
8. Refers patients to relevant patient care services in the community as appropriate. Advocate on behalf of patients to obtain services as necessary
9. Provides education and support to patients and their families to encourage compliance with medical and behavioral regimes

11. Documents all appropriate information in the electronic health record system
12. Responds to messages and relay information to patients and other external sources as needed. Write letters documenting and/or summarizing diagnosis and treatment as requested and appropriate
13. Submits Treatment Authorization Requests to insurance companies as appropriate. Complete various other medical related forms as needed
14. Assists in the planning and facilitation of educating providers on behavioral health topics
15. May participate in and collaborate with staff/teams on various support-related functions including quality assurance, care team development, system optimization, empanelment, policy and procedure development, etc.
16. Works with Behavioral Health Manager to facilitate administrative aspects of the program.
