POSITION TITLE:              Behavioral Health Program Navigator

LOCATION:                           West Sacramento, CA

Salary: $23.32 to $28.57 hourly 

*** This position requires traveling to Napa for training during the first few weeks***

JOB SUMMARY: The BH Program Navigator is a member of the Behavioral Health team and works in continuous partnership with the patient, their family when appropriate, BH providers, OLE Health care team, and relevant community partners to improve health outcomes and meet the patient’s desired health and wellness goals by:

  • Serving as the contact point, advocate, and informational resource for patients, family/caregiver(s), behavioral health providers, payers, and community resources
  • Facilitating access to internal and external services
  • Identifying and addressing gaps in care
  • Increasing patient’s ability for self-management and shared decision-making

EDUCATION, EXPERIENCE, TRAINING

  1. High School Diploma or equivalent. Undergraduate degree in social services preferred.
  2. Experience working with diverse populations; displays respect and sensitivity to cultural diversity.
  3. Experience working in community health setting.
  4. Strong collaborative skills; ability to effectively and appropriately engage members of the care team, other clinic departments, leadership, and external community partners.
  5. Exceptional communication skills; ability to effectively engage patients and assess needs and concerns to ensure that the patient is referred appropriately.
  6. Excellent in-person and phone customer service skills; able to manage difficult and emotional patient situations and to respond promptly to patient needs.
  7. Ability to adapt to change; approach challenges with an innovative and proactive attitude; and investigate resources.
  8. Strong verbal and written communication skills required; Bilingual (English/Spanish) required.
  9. Must certify and remain current in CPR.

DUTIES AND RESPONSIBILITIES

 
1. Evaluates social determinants of health, identifies barriers to care and addresses/resolves obstacles impeding progress
2. Provides support to help maximize quality measures
3. Determines the appropriate level of behavioral health services needed to support the patient at any given point in time
4. Facilitates patient access to appropriate behavioral health providers and services, both internal and external
5. Manages behavioral health schedules to improve access to services
6. Develops a relationship with community partners to ensure robust collaboration
7. Participates in huddles, team meetings, and case conferencing as needed.
8. Educates patient and family/caregiver(s) about relevant community resources
9. Acts as point of contact for any behavioral health need
10. Collaborates with general care coordinators to limit overlap and ensure closed-loop BH referrals
11. Provides support to behavioral health providers by completing forms and other tasks as needed
12. Documents timely and efficiently in the EMR
13. Attends appropriate training courses/webinars and meetings
14. Gathers and reports data related to behavioral health when needed
15. Provides feedback for the improvement of behavioral health processes
16. Perform other duties as assigned
