We serve everyone at OLE Health, even if you can’t afford to pay. We have a sliding/reduced fee scale based on family income.

Payment and Billing

Payment is due at the time of service, but patients will not be turned away because they are unable to pay. If you don’t have insurance, we offer payment plans in addition to a sliding fee scale, meaning we will work with you on an amount you can afford.

For the sliding fee scale, bring your proof of income for the current or prior month with you to your appointment. Proof of income can include your most recent paystub or a signed statement of your income from your employer if you work in cash. Click here for a full list of acceptable documents.

OLE Health understands that insurance coverage can sometimes be challenging to obtain and understand.

Our staff can help you:

  • Find out if you qualify for our sliding fee scale
  • Understand your options under the Affordable Care Act (Obamacare) and help you find, enroll in, or renew coverage

Proof of income should be for the current or previous month. This can be your latest pay stub or a signed statement of your income from your employer if you work in cash. By registering for this event, I authorize OLE Health to use my picture, including photographic and video images, and my voice. I hereby grant to OLE Health the right to use, publish, and reproduce, for all purposes, my name, pictures of me in film or electronic (video) form, sound and video recordings of my voice, in printed and electronic copy in any and all media including television and the Internet, and for exhibition, promotion, advertising, press conferences, meetings, conferences and in brochures and other print media. I hereby release OLE Health and any of its associated or affiliated companies, their directors, officers, and employees from all claims of every kind on account of such use.

If I am a minor under 18 years old, a parent or legal guardian is required to read this release and approve its terms.
